Disability & Carer Services

The introduction of the NDIS across NSW has given you the opportunity to choose disability services that may have been unavailable before now. This new ability to choose services might also have bought a degree of stress and confusion as you try to navigate support plans and access the services you desire.

Community Links Wellbeing understands these difficulties. We are here to give you an obligation free helping hand to understand the NDIS and get your supports in place. You do not have to take up our service after receiving information from us. Our service has been registered as a provider with the National Disability Insurance Agency and is third party verified by the Institute of Healthy Communities. This means we can assist people with disabilities who have funding approved for the following services:

  • Support Coordination
  • Individual Counselling
  • Behaviour Support
  • Therapeutic Supports
  • Early Childhood Supports
  • Assist-Life Stage, Transition
  • Speech Pathology
  • Occupational Therapy
  • Exercise Physiology

  • If you are confused and overwhelmed by the NDIS and need assistance to understand what it is all about then you can call us for information and support with no obligation
  • If you have your approved NDIS plan, you or your support coordinator can contact CLW to arrange a meeting
  • CLW will assess whether we can provide the services you are requiring
  • CLW will prepare a Service Agreement  for the services we will provide for you
  • You will be contacted by a staff member to book in the services we have agreed to provide for you
  • We will then bring your plan to life in line with your requirements. For example; you may have funding in your plan for Improved Daily Living. We will explain what Improved Daily Living is, how much you are funded for, what types of activities this includes and how many hours you have been funded for. We will then link you into the services to make this happen.

We are able to provide a range of resources and planning so you, the Carer of someone with serious mental health concerns can take care of yourself. Carers need quality time out and the five organisations who form the Haven Project have specific skills and an understanding of the individual Carer’s needs and wants.
